**The Parent or guardian needs to present a copy of their child’s shot records, and any letters received from your child’s school district.

If your child has had shots previously at the City of Kansas City, Mo.,

Health Department, you can get a copy when you register.



**In accordance with State of Missouri Law students are not allowed to attend school until the school district has satisfactory evidence on file that the student has been immunized, or, that the students is exempted from obtaining immunization.

(OPEN TO ALL SCHOOL DISTRICTS AT NO CHARGE)



**Daily hours of operation: Mon, Tues, Wed, & Fri- 8:00-4:00, Thurs 8:00-noon**
